Serenity offers both home health and home care services to meet the unique needs of our patients and families. While both take place in the comfort of your home, they serve different purposes:

  • Home Health is provided by licensed medical professionals (RN, PT, OT, and SLP). It focuses on skilled medical care — such as wound care, rehabilitation, and other medical and therapeutic services — to support recovery and health management.
  • Home Care provides non-medical support, including assistance with daily activities, companionship, and personal care. These services help clients maintain independence, safety, and quality of life at home.

All enrolled Veterans may be eligible for Homemaker Home Health Aide services if they qualify for community care, meet the clinical criteria, and the service is available in their area. Services may vary by location.

Long-term care (LTC) insurance helps cover the costs of extended care for individuals who need ongoing support. This may include in-home care, assisted living, or other services that help with daily living and safety.

LTC policies are separate from standard health insurance and require monthly premium payments. Coverage varies by plan and payer.

At Serenity, we partner with clients and families to offer home care services on a private pay (self-pay) basis when preferred or when services aren’t covered by insurance, Medicaid, Medicare, or other third-party payers. Private pay care is flexible and fully customizable, allowing us to design support that fits your unique needs, goals, and schedule.

Yes. At Serenity, we understand that every condition requires a thoughtful and compassionate approach. We provide individualized care for clients living with conditions such as, but not limited to, dementia, Parkinson’s disease, and those recovering from surgery. Serenity provides care seven days a week — including weekends and holidays — according to the hours approved through insurance or outlined in each client’s individualized care plan. We work closely with clients and families to ensure reliable, consistent support during approved hours, including 24/7 care when needed.

Our office is open Monday through Friday from 9:00 a.m. to 5:00 p.m., and we also offer an after-hours assistance line at 773-588-4000 so help is always within reach.

Yes. Our caregivers can provide safe transportation as long as they hold a valid driver’s license and active insurance. They can help clients run errands or — if preferred — drive the client in the client’s own vehicle to attend appointments, run errands, go out for lunch, and more. If transportation is something you need, simply let us know and we’ll make sure it’s built into your care plan. Along with being trained and skilled, our caregivers focus on assessing personal needs and offering meaningful companionship every step of the way.
